Staff roles who should complete it include direct staff, non-direct staff, supervisors, directors, and senior leadership, with 100 percent completion expected for certain IDHS or DCFS-funded programs. Responses use present, emerging, not present, or limited not applicable choices, and comment context is encouraged, especially when selecting not applicable.
