A Saint Luke’s Foundation grant agreement is available for signature.” a. Read the email carefully. b. When ready to access the agreement, please click on “Saint Luke’s Foundation online dashboard” in the first paragraph of the email c. Alternatively, you can visit saintlukesfoundation.org and click Apply in the upper-right side of the screen. 2) If you are not already logged in, enter your email address and password, then click the blue Log On button 3) Scroll down to the bottom of the screen with the brown bar that says “Form Name” a. On the righthand side of the brown bar, you will see Edit/View. Click Edit right below it. 4) This is the Grant Agreement. a. Read through the grant agreement carefully b. If you agree with the agreement, please select “I Accept Grant Terms and Conditions” under the Acceptance of Terms and Conditions question. c. Type your name under the Authorized Signature question. d. Type your title under the Title question. e. Input today’s date under the Date question. f. Please note the bank ACH Payment Process so that we can pay your expenditure grant electronically. To download the form, click “visit this link” under the ACH Payment Process instruction. We will not be issuing paper checks. g. Alternatively, you can visit https://www.saintlukesfoundation.org/files/assets/achcombinedformnewcontactinfofillable.pdf to download the form. 5) Scroll to the bottom of the screen and click the blue Submit Follow Up button. Upon submitting your grant agreement, you should reach a screen that says “Your Follow Up has been submitted.” You should also receive an email acknowledging your grant agreement submission. Reminder: Please ensure that you have followed the instructions on the ACH form, completed it, and submitted it along with a voided check to grants@stlukesfdn.org.
