We utilize trained care navigators from the John Hopkins team to conduct comprehensive assessments, including the PROMISand ZARIT screenings, to establish a baseline for care. After submitting our findings to Medicare, we create customized care plans based on the detailed needs assessment we conduct with both the patient and caregiver.
