I covered what to fill out in Sections 1 to 4, including the plan custodian details, owner information, the account number, the full transfer amounts, and checking separated from employment for an old employer 401k. I also noted we must have a wet signature on the form. New business will review the e-application first and not initiate the transfer until they receive the 9685A.
