I show you to go to the Questionnaire tab, click Add Questionnaire, and optionally upload a cover image sized 1440 by 600 pixels. Then I add basics like name, description, and an optional expiration date, and click Save. Next, I build it by adding sections and question types, setting required fields, editing with the pencil icon, and reordering questions. I finish by clicking save so it appears in your list, ready to use.
