I show how to add the narrative, upload evidence like images and an SDS, and add multiple witness statements including the injured party and supervisor. Then I complete a 5Y analysis and write a root cause statement about a lack of a defect reporting system. I generate a findings summary and create a safety issue with corrective actions added to the issue register. I assign the follow up inspection of all chemicals due by tomorrow, and ask the WHS manager and ops manager to sign off.</description></oembed>
