I show how to report an injury incident from the dashboard, select near miss and damage options, and enter incident summary details. Using the sample data, we record a short term injury requiring treatment, the injury location, chemical burns to the cheek, and a face body region, plus witness details. I also set up the required investigation, assign myself as lead investigator, and add an immediate hierarchy of control with corrective actions like withdrawing a pump from service and doing checks.
