We specifically address situations where an individual&apos;s fluid intake falls below the target healthy baseline, and I outline the steps to create an Action Plan to prevent this from happening again. Once the Action Plan is entered into the system, we can acknowledge the alert, which will then be documented as a progress note in the resident&apos;s profile. I encourage all care staff to ensure this information is shared during handovers and is accessible via the mobile application at the point of care.
