Open the contact either from the pinned Contacts app or from Conversations, then tap Notes, hit the plus sign, add your note, and click Save. You might not see it right away, that is normal. Touch and press and hold to refresh, and the note will show up, or go back into the contact and refresh it. If needed you can delete or edit the note. No action is required except to try it.
